How To Find The Right Community

Whether you need temporary rehab care or a caring place that meets your lifestage needs, NHC offers a community for you.  At NHC, we strive to make every moment count for our residents and patients because we believe in celebrating life at all stages.  We want each resident and patient to feel supported and connected.  Let us find the right community for you.

SKILLED NURSING

Skilled nursing provides a bridge between home and hospital, with 24-hour nursing care provided by licensed nursing teams. Whether provided for several weeks or several months, our physician directed skilled health care services are designed to complement a hospital stay.

CONTINUING CARE

Continuing Nursing Care is for patients who require continual nursing care.  Patients in this area receive 24-hour nursing care and supervision for support in their daily activities, which includes help with bathing, dressing, meals, incontinence care, mobility and managing medication. These patients, through our care, are able to achieve and maintain the highest possible level of function.

REHABILITATION

Rehabilitative Care surrounds patients with support – friendly faces, helping hands and skilled therapists committed to helping with every step of recovery. Providing compassionate post-surgical care, we work with patients and their physicians to arrange a comfortable transition to therapy and rehabilitation.  Our rehabilitative care includes physical, occupational and speech therapies.  Our experience includes orthopedic care for knee and hip replacement and cardiac and stroke recovery. 

ASSISTED LIVING

Assisted living and residential living provide thoughtfully designed private living spaces, inviting community areas and daily activity schedules that contribute to the feeling of home.  We create personalized plans for each resident’s health needs and preferences.  Licensed medical supervision provided on-site 24/7 adds to a feeling of safety and security. 

INDEPENDENT LIVING

Independent living means enjoying a new way of living. This is the perfect option for those individuals, 62 years and older, who wish to maintain active, independent lifestyles without any home maintenance worries. Our private residences are located in a quiet neighborhood, with access to activities and meals in our nearby senior living centers. 

MEMORY CARE

Our memory care centers provide a comfortable and protected world where patients with moderate memory impairment can live a lifestyle that provides a genuine sense of home and belonging.  Patients have a beautiful, secure and caring environment free from daily burdens.  Our centers are fully secure, with highly trained medical staff, emergency response systems and personal assistance with medication and personal care. 

RESPITE CARE

Respite care is available at all of our skilled nursing facilities.  Respite is short-term or temporary care services to relieve an in-home caregiver of responsibilities for an individual with long-term care needs. Respite care provides short-term breaks that can relieve stress, restore energy, and promote balance in your life.  Respite patients receive quality care, spend time in a safe, supportive environment, interact with others and if able, participate in activities designed to match personal abilities.

HOME CARE

If your physician decides that home health care is right for you, after an injury or illness, NHC Home Care is here for you. Our experienced team of nurses and therapists are dedicated to provided our patients with the knowledge, skills and care they need to recover, take charge of their own health and be able to remain safely at home.

BEHAVIORAL HEALTH

NHC is proud to announce the expansion of its behavioral health service line. This year, NHC will open two new centers focused on providing psychiatric care to patients who require inpatient hospitalization due to mental disorders, including cognitive illnesses.

Care Beyond Your Stay

NHC HOMECARE

NHC HomeCare programs assist those who wish to stay at home or in assisted living centers but still require some degree of medical care or assistance with daily activities. Registered or licensed practical nurses and therapy professionals provide skilled services such as infusion therapy, wound care, and physical, occupational and speech therapies. 

HOSPICE

Hospice provides compassionate care to individuals in the final stages of life.  The focus is managing pain and making you or your loved one as comfortable as possible by responding to physical, emotional and spiritual needs with dignity and respect during the last stretch of life.
